重点学习了第十章。对现行医学教育的问题，如基础与临床的割裂，轮转的不系统，对终结性评价的过多依赖等，把脉比较准。但是作者对医学教育系统的持续的sustaining improvements 显然是低估了。事实上教育界是比较早的意识到这些问题的，先后进行了以系统整合为特征的改革和目前以岗位胜任力为导向的改革，提出了系统性的解决方案。模拟技术正在日益广泛的得到应用，但是也暴露出人文素养的缺乏。作者提到的现行毕业后医学教育、准入和认证体系是医生执业的法定程序，corporate medical school这种小作坊生产的产品是无法适应这种质量评估体系的，这或许是最难disrupt的。
